How to Use This Template

Print the ABC template and place it in an easily accessible location, such as on a clipboard within arm's reach of the learner. Document each episode immediately after it occurs. Record the precise date and time of the incident, then write objective, observable descriptions of what happened immediately before (Antecedent), during (Behaviour), and after (Consequence) the episode. Use the notes section to capture setting events like sleep disruption, illness, or changes in medication.

Clinical Best Practices & Tips

Keep descriptions strictly objective: focus solely on what you saw and heard, avoiding subjective guesses about the student's inner feelings, motives, or intentions. Logging data as close to the incident as possible is critical, as memory bias significantly drops data accuracy over time. Keep your notes brief and use abbreviations where possible so that tracking does not interfere with instruction or active support.
