How to Use This Template
- Fill out the student's name, target behavior definition, and start date.
- Write dates or days of the week across the column headers (e.g., Monday through Friday).
- Whenever the target behavior occurs, record a tally mark (e.g., |) inside the box corresponding to the current time interval and day.
- Leave the block blank if the behavior did not occur during that interval.
- Sum the tallies at the end of each day or subject interval to calculate total frequency counts.
- Analyze the sheet after 1-2 weeks to identify horizontal clusters (high-frequency times) or vertical clusters (high-frequency days/routines).
