Printable Frequency Tally Counter Sheet

Reliable frequency data starts with a simple, uncluttered recording sheet. This portrait-format tally grid gives observers five labelled behaviour rows and ten session columns — enough to track a full school week at two sessions per day or any custom interval schedule. A totals column and totals row make end-of-day summarising fast, keeping the cognitive load on behaviour analysis rather than arithmetic.


How to Use This Template

  1. Fill in the student identifier, observer name, date range, and approximate session length at the top.
  2. Write each target behaviour's operational definition in the 'Behaviour' label field at the start of each row.
  3. Label the column headers with session dates, times, or interval numbers (e.g., Session 1, Monday AM).
  4. Each time the target behaviour occurs, add a tally mark in the corresponding cell.
  5. At the end of each session, sum the tallies in that column and write the total in the bottom 'Session Total' row.
  6. At the end of the recording period, sum each row's tallies and write the total in the right-hand 'Total' column.
  7. Use the totals to calculate rate (count ÷ session length) for comparison across sessions.

Clinical Best Practices & Tips

  • Define operationally: Ensure all staff recording data are using an identical, observable definition of each behaviour before starting. Ambiguity inflates inter-observer disagreement.

  • Cap at five: Tracking more than five behaviours simultaneously introduces errors. If you need more, use separate sheets per observation focus.
